1. Do I need to fill out any forms in advance?
All new patients are required to fill out a health history form so that we will be aware of your medical history. This form is available at our office.
2. What do I need to bring on my first visit?
Please bring any proof of identification and insurance information. If you have a history of being diagnosed or treatment, any X-ray, MRI, CT report, or any relevant document that will help us assist you better will be a great help. If you do not have this in your possession, you can request a copy from your attending physician or from the practitioners you have previously consulted.
3. What clothes should I wear when I visit for the first time?
Please wear appropriate clothing so that our chiropractors can comfortably examine your body and properly diagnose your condition. Our chiropractors will perform tests to detect any irregularities in your muscles, nerves, and skeletal system. Sports bras for women, bike shorts and other comfortable clothing are advised.
4. Will I be treated right away?
After your examination, our chiropractors will need to go over your test results. The practitioners will recommend the appropriate chiropractic program for you. It is best to have someone with you (spouse, partner, parent, or guardian) during the report of findings. Any questions regarding your program may be addressed to the practitioner during this time.
5. Do I need to pay right away?
We will advise you of financial arrangements and insurance coverage if applicable.
